The lava substrate is quite widely used here, but I'd imagine there are many different sorts, depending on where it's from.
I haven't heard of any issues of it causing high phosphorus on here though.

It's normal to need to top up iron/epsom salts.

I'm still doubting if there is a phosphorus issue if the lettuce aren't bolting.

What's your fish load like?
